Maybe it's after an autopsy? Since he IMMEDIATELY explains what happened, his "he's been shot!" line may just be him being a smart-ass. So when Batman relates he hit him hard in the chest (presumably earlier off-panel, in describing what happened), would there be a quick check the doctor could do even in 1942 (X-ray?) that indicated yup, the bullet went into the heart, and that leads to the third panel.
